Applications and Forms

• Print and fill out a copy of the Membership Application.  Send the original form along with a legible, notarized copy of your current state issued driver’s license or other government issued ID and a check or money order for at least $10.50.  Please provide you full social security number on this form.
  
• Print and fill out a copy of the Checking Account Application.
  A minimum monthly deposit of $100.00 must be made to the checking account by Direct Deposit or ACH.  Complete the appropriate form, located on this page, and include it with the checking account application.  When providing your social security number, please give just the last four digits.
  
• Print and fill out a copy of our Debit Card Application.  An active checking account is required to apply for a debit card.  When providing your social security number, please give just the last four digits.
  
• To apply for a loan, go to FlexTeller, log in to your account, select the Loan Application tab, and complete and submit the application.  For future loans, all you will need to do is edit the existing information for accuracy and updates.                                          
  
• Direct Deposit Authorization Form.  If you are paid by Ecolab Inc., go to www.ecolab.com/myhr  to set up the direct deposit for any amount that you want going to savings or checking.  Use this form if you have a loan payment.
  
• ACH Authorization Form.  If you are not paid by Ecolab Inc., funds can still be deposited to your savings or checking account or payment made to an Ecolab Credit Union loan.  Complete this form, attach a void check from the account that you want us to pull the funds from, and email, fax, or mail it to us.
  
• Print and fill out a copy of the Address Notification if you change your address or phone number.
  
• Western Union is available for those needing immediate cash.
  
• Wire Transfer Form.  Use this form to have money sent to another financial institution from your Ecolab Credit Union account. When providing your social security number, please give just the last four digits.
  

All forms except the original Membership Application may be emailed, faxed, or mailed to Ecolab Credit Union.
